Addition to the payment pages

I'm working on a web site for Equine products, we need a check-box on one of the payment pages (preferably one of the first couple) to say that the customer is an owner of horses and the products being purchased aren't for resale. The box should be set so that it needs to be checked to progress past that page.

This needs to appear on the data entry report printout.

In Design / Text / Web Site (cont) / Invoice Address there is a field "User Definable" if you check this you also get an additional checkbox "Required". Check both of these and change the Prompt text to be whatever you need.

p.s. There's also a similar field in Design / Text / Web Site (cont) / Shipping &Tax called "Taxes User Definable" that you could use if that's a more logical place.

Do you know how to get it to appear as a checkbox?
At the moment it's a text box which may get confuse the customer as to what they are supposed to enter.
