Verifying a customer agreed to special terms of booking

I need my customers to read and agree to the terms and conditions of booking a course on-line. How can I verify that they have agreed to this.?

If possible I need also link to the document that will be in PDF format that pops up on a completely serarate page.

A tick box would then be sufficient that they can tick once they have read and agreed.

Is it also possible that the form when completed by the customer can be forwarded in with the order.

You can embed a link to a PDF document just by editing the template mentioned in the article. I don't think you can forward this with the order though - customers will have to email it separately.
